MEDICAL AND LIABILITY FORMS
All Faith Formation students in Grades 3-9 need to submit the Multiple Event Youth Forms with Medical and Liability information if they have not done so already. Parents sign their child in at events on this form. Just download the forms with the button below. Bringing a friend to an event? They need forms, too, unless a parent attends with them.
