You can complete your family's registration on-line below or download the form for the 2019-2020 St. Gall Faith Formation Program. You will also see a Family Information Form to update and confirm your contact information and the Program Overview for the coming year. Pick up your Family Handbook, your child's class folders and calendars from the Faith Formation Office. Parent Curriculum Nights are during the first class. Watch the bulletin, parish website and our Facebook page for more details, registration and forms for events.
It is very important that families are actively involved in the faith life of the parish, especially attending Mass on a weekly basis. Everything learned in Faith Formation flows to and from the Sunday (or Saturday evening) Liturgy.
Thank you for allowing us to partner with you as you teach your children about Jesus and our Catholic faith!

Tuition and Fee Policy
St. Gall requires parents to pay fees in order to help defray the costs involved in providing your children with faith formation. All fees are DUE with your registration on or before AUGUST 1 unless arrangements have been made for monthly payments. There will be a $35.00 late fee assessed for those registrations received after August 1 and a $50.00 late fee for registrations received after September 1. The late fee will not be assessed to new families joining the parish after August 1st if fees are paid at time of registration.
In circumstances of true need, St. Gall considers it an honor to provide financial assistance. All requests will be kept in the strictest confidence. Please contact the Director of Faith Formation at 630-365-9166 for more information.
We also have an Adopt-a-Student Program for those wishing to donate any of the following amounts below. Please know that no amount is too small and your generosity is greatly appreciated.
Our Faith Formation Programs are only successful because of its volunteers. You can make a tremendous difference in the faith lives of our children and teens. Each family is asked to volunteer in at least one area. Please select at least one area in which you can assist even if it is small! More opportunities will be available as the year progresses.
Catechists and Aides: We appreciate your time and dedication. St. Gall does not charge a tuition fee for the students of Catechists and Aides. All Catechists and Aides are expected to be in the classroom each week, MUST attend weekly Mass and work towards Catechist Certification, attend In-Service classes and have completed the Protecting God’s Children program and other forms. This is a very rewarding ministry!
Parking Lot and Outdoor Safety: The Outdoor Safety Volunteers ensure the safety of the children as they enter and leave St. Gall. In appreciation and thankfulness to those who stand in the rain, snow and freezing sleet, get to know each child, are a great witness to service and learn which car they ride home in each night, they receive 30% off of their children's tuition fee. All Outdoor Safety Volunteers are expected to be at St. Gall no later than 15 minutes before class begins or ends, MUST attend weekly Mass and be certified in the Protecting God’s Children program before assisting in any of our programs or service projects. Please keep in mind that all students in Grades K-4 should be picked up by their parent (or an approved older sibling) at the end of each class. Please be courteous and observe the routes in the parking lot for cars to ensure everyone’s safety. Thanks!
Office Assistants: Volunteers are needed during the Wednesday class times for all kinds of support for our Catechists and classes. They will receive free tuition.
Please Note: All catechists and volunteers must go through the Diocese of Rockford's Protection God's Children and Safe Environment Program. Information on these programs can be found below.

MEDICAL AND LIABILITY FORMS
All Faith Formation students in Grades 3-8 need to submit the Multiple Event Youth Forms with Medical and Liability information if they have not done so already. Parents sign their child in at events on this form. Just download the forms with the button below. Bringing a friend to an event? They need forms, too, unless a parent attends with them.

You only need to submit this form once. If you have done it already, thank you!
We need permission to publish pictures for all children under 18 years of age in different formats. Please fill out a form for your children and submit to the Faith Formation office. We would love to show the amazing things that are happening at our parish on our web site, Facebook pages and other social media. Thank you!
